Insurance and Billing Policy

It is your sole responsibility to provide us with your current insurance at the time of service. Your current insurance card and copayment need to be provided to us at each and every visit.

Copayments are due at time of service. Copayments not made at time of service are considered past due and are subject to late fees and interest.

Any amounts that are deemed the patient’s responsibility by your insurance are due upon receipt of your billing statement. Any amounts not paid within 30 days are subject to late fees and interest.

Any account that passes 90 days is subject to referral to a collection agency without notice.

Acceptable forms of payment are Check, MasterCard, Visa, and Discover.

We accept MasterCard, Visa, and Discover over the phone during business hours.

Missed Appointment Policy

In order to give every patient the time they need for their healthcare we require 24 hours’ notice any time you are unable to keep your appointment. This will allow us to cancel your appointment and use the time for another patient. Call our office in advance and we will be happy to cancel and reschedule another appointment for you. Our voicemail is available to take your message 24 hours a day. If an appointment is missed, we will charge you a nominal fee for the time that was not utilized. If you continue to miss appointments without cancellation, we will ask that you find another physician for your healthcare needs

How Well Visits Are Billed

Sometimes at a well visit it may be necessary to discuss issues that are outside of a normal well child visit. (ex: cough, cold, asthma, chronic issues, medicine refills, etc.)

Please be aware that if there are any issues like these discussed during the well visit we will bill your insurance company a well and an established (sick) visit.

If you have a deductible or a copay your insurance may assess a charge for these. If this is the case, we will send a bill in the mail to you.
